To register for the 2019 Australia and New Zealand Rhinologic Society Conference, please follow the steps below:

- Ensure the "Start Registration" circle is selected
- Select the number of delegates you would like to register, 
               - Please enter one (1) if you are only registering yourself.
- Enter the names of all delegates in your group below
- Then click "Next" to begin the registration process.

Please Note: You will be required to complete the online registration process for each delegate.

To complete the details on each page:

1. read each section carefully and only fill out information as instructed - all information should be typed in sentence case (e.g. John Smith)

On submission of your registration:

1. your information and credit card details will be encrypted and securely forwarded to Consec - Conference and Event Management

2. you should receive an automated response of submission within 5 minutes which will have your receipt and confirmation letter attached

3. if you have NOT received a confirmation letter and receipt within an hour of registering please contact Consec - Conference and Event Management at anzrs@consec.com.au or call +61 2 6252 1200

Please note:

1. the automated response after your submission means your details have been processed and your credit card has been charged.

2. Consec - Conference and Event Management will be onsite from Friday 20 September 2019. Contact to the Consec office regarding the conference must be made before this date.

3. registrations will NOT be accepted to the office from midnight on Friday 13 September 2019, with registrations being available onsite from Friday 20 September 2019.

4. attendees are advised that photographs may be taken during the conference and reproduced for promotional purposes.

Privacy Policy As a conference delegate, your name and contact details may be published either for distribution to fellow delegates or to another party. Please indicate on the Data Processing Consent page if you do not wish to be included in the delegate list by ticking the "No, don't put my name on the delegate list" button.

The Welcome Reception and the Conference Dinner are inclusive for the Member, Registrar and Non Member registration types only.

Terms and Conditions
Please ensure you have carefully read the terms and conditions. By registering for the conference you accept these terms and conditions. If you do not understand any part of the terms and conditions, please do not hesitate to ask us for more information. <u>Conference Cancellation and Refund Policy</u> Delegates who find themselves unable to attend the conference after submitting a registration form are most welcome to nominate a substitute. In the case of a cancellation, delegates must inform Consec - Conference and Event Management in writing of their inability to attend. A refund of the registration fee, less a cancellation fee of $110.00 (AUD) including GST will be made on or before 20 August 2019. NO refunds will be made to anything after the 20 August 2019. All refunds prior to 20 August 2019 will be made after the conference. Any changes made to a registration within seven (7) days of the conference will incur a $40.00 administrative fee. Accommodation Cancellation Policy Delegates who cancel their accommodation booking must inform Consec - Conference and Event Management in writing of their cancellation before 18 August 2019. Bookings made after that date cannot be guaranteed. Please check the individual hotels cancellation policies. Please DO NOT contact the hotel directly. Disclaimer The conference program and registration information is correct at the time of viewing/printing. However, the organisers reserve the right to change the information where necessary without notice. Attendees are advised that photographs may be taken during the conference and reproduced for promotional purposes.
